Patients diagnosed with VTE in the ER should receive their first dose of LMWH in the ER prior to discharge.
Apixaban is the DOAC of choice unless patient has a medication contraindication. Dabigatran or Rivaroxaban are no longer considered first line.
Patients taking estrogen who are diagnosed with VTE should continue to take estrogen as long as they are taking their DOAC as well.
Since ddimer represents hyper coagulability - it is reasonable to refer someone to the thrombosis clinic if their imaging is negative but their dimer is high (and especially if they don’t have primary care follow up).
